Closing the gap between health advice and real-world participation.

Physical activity is central to prevention, recovery, long-term health and quality of life. However, the journey from receiving health advice to participating safely and consistently is often fragmented.

Medical professionals, fitness professionals, digital platforms and community organisations may each support part of the journey, but they do not always operate as a connected system.

The BK Sports Medicine Active Health Innovation Ecosystem explores how these areas can work together more effectively. The aim is to create clearer pathways from assessment and personalised recommendations through to safe exercise delivery, progress monitoring and sustained community participation.

A structured medical pathway that considers health, lifestyle and physical activity together.

The BK Sports Medicine Precision Lifestyle Medicine Program is led by Dr Bikram Karmakar and combines Sport and Exercise Medicine, Lifestyle Medicine and precision health technology.

The program considers exercise, nutrition priorities, sleep, stress, recovery, prevention, medical history and individual health goals together. Recommendations are prioritised according to the person's needs, medical background and what is realistic for their life.

It is designed to support people who require more than generic lifestyle advice, including those managing metabolic risk factors, complex health priorities or barriers to becoming more active.

It considers the interaction between training load, physical capacity, biological factors and current clinical concerns. The result is a structured risk profile with safeguards tailored to the factors identified.

The tool is intended to support informed decision-making and safer participation. It does not replace clinical assessment, diagnosis or individual medical advice.

Outcomes and evaluation

Developing better ways to understand whether interventions are producing meaningful improvement.

Velyra is a digital outcomes platform currently in development.

The platform is being designed to collect and track patient-reported outcome measures across episodes of care. Its intended purpose is to support individual progress monitoring, clinical audit, service evaluation and research-ready data collection.

Velyra represents the measurement component of the wider ecosystem. It explores how services can move beyond recording what was delivered and better understand what changed for the patient over time.

The ecosystem is not a single program that every person must complete. It is a portfolio of connected initiatives addressing different parts of the active health journey. Together, they demonstrate how medical care, fitness services, digital tools and community participation can operate with greater continuity.

Dr Bikram Karmakar is a Specialist Sport and Exercise Physician and Lifestyle Physician with experience across clinical medicine, elite sport, education, governance and the fitness industry.

His work focuses on improving the connection between healthcare, physical activity and professional exercise delivery. The Active Health Innovation Ecosystem brings together clinical services, digital platforms and community initiatives developed or led across these areas.
